Request for Faculty Authorization
The Circulation Department of Mullins Library allows you to authorize faculty assistants (graduate students or staff) to check out library materials for your use and/or to pick up Interlibrary Loan materials. Undergraduate students may be granted authorization only if they are employed by the faculty member.
Individuals authorized will need to present their University ID in order to check out or pick up materials for the faculty member.
If authorization is granted, the Circulation Desk will be unable to distinguish between materials checked out to you or your assistant(s). All items will be checked out to you. All notices produced by the circulation system for materials checked out using the authorization procedure will be sent to you, and not to the individual authorized.
If you prefer, authorization may be given for Interlibrary Loan pickup only.
Faculty authorization may be issued for a maximum period of one year (may be renewed annually). You will not be contacted when the authorization expires. You may authorize a designated student or staff member by completing the attached form. A maximum of two individuals may be authorized.
